Track Performance Over Time
What is the best way to track the performance of a specific service over time?
Imagine, for example, that your organization is trying to improve Depression Screenings.
What do you know about the number of Depression Screenings you have been able to perform in the past, and how many screenings will result in a significant improvement in the future?
That is what your Check Dashboard does for you!
The Check Dashboard opens with a series of tabs at the top of your screen. The tabs you will find are Services, Revenue, Eligibility, Scheduling, Quality Performance, and TCM Invoices.
View Services over Time
To track performance in a particular service over time, click on the “Services” tab. In this view, you will find completed services in a graph view on the top, and a list view on the bottom.
In the graph view, you will find two dropdown menus on the right-hand side. From these menus, you can filter your graph view by date and service.
Filter by Service Type
So, if you want to track Depression Screenings performance across your patient population, simply click on the Filter By Service dropdown menu and select Depression Screenings. You will notice that your graph view now focuses on past performance that specific service!
For those seeking to see results for Depression Screenings for a specific period, the “Filter by Date” dropdown menu is available.
The Services view of your Check Dashboard makes it simple for you to instantly view the information that matters to your organization. After taking a closer look at your past performance in specific services, you will be able to better understand how to improve performance in the future.
View Reports and Post-AWV Care Plan
Check users know that the platform makes Annual Wellness Visits easy and painless for providers and staff.
However, did you know that Check automatically creates a post-AWV care plan, patient report, and provider report?
What’s the big deal with these reports, anyway? Why do they matter?
After an Annual Wellness Visit, Check generates three reports: Full Results, Provider, and Patient.
Here’s a breakdown of the information in each report:
- Full Results: This report contains an overview of every question and answer during the Annual Wellness Visit. As well as providing the most comprehensive look at a patient’s current health status, the report also serves as a complete record of an Annual Wellness Visit when submitted to CMS. This documentation allows you to attribute patients to your practice in PCF and is proof of the Annual Wellness Visits your providers are completing. It is also a great report to attach to the patient record in the EHR.
- Provider: This report is a summarized view of each aspect of the Annual Wellness Visit. Providers can access this report to gain a quick overview of patient health. This report also includes notes from previous services and a list of services that should be performed in the Check platform.
- Patient: This report is a detailed view of the patient’s current health situation, designed to be printed and given to the patient after an Annual Wellness Visit. This report contains an overview of possible health risks, facts, activities, and helpful tips. Using information provided by the patient during an AWV, this report is intended to help the patient better understand their health. When given to the patient, this report acts as a helpful guide for the patient to improve their health situation and keep an eye on risks.
What’s the best part of all these reports? Check generates them automatically based on the information provided in an Annual Wellness Visit!
These reports allow you to document services, gain a better picture of patient health, and provide patients with helpful information — and Check does all of this while saving providers time and effort.
Here’s how you can access the reports:
- By searching for the patient’s name in the search bar on the left-hand side, you can find reports for any patient. Once you access the patient’s information, you can select the “Completed” tab under the patient’s name. In this section, you can view the services your providers have completed with the patient and access the relevant reports for those services.
- Reports can also be accessed through the “Services” tab on the left-hand side of the screen. Select “Completed Services” to view a list of all completed services completed by any member of your organization. You can filter this list by individual services and access reports created once the service has been completed.
