Details from the 2027 Proposed Rule | Ask Dr. Mingle

CMS released the Calendar Year (CY) 2027 Medicare Physician Fee Schedule Proposed Rule on July 14, 2026, and there’s a lot for clinicians, practices, and ACOs to unpack.

In this episode of Ask Dr. Mingle, Dr. Dan Mingle walks through the biggest changes proposed for QPP, MIPS, MVPs, and MSSP, and what they mean for your reporting strategy heading into 2027 and beyond.

 

CMS Proposed Rule for the Physician Fee Schedule for 2027 (QPP, MIPS, MSSP, & more)

In this episode, Dr. Mingle gives a comprehensive rundown of what’s in the CY 2027 Proposed Rule, including:

  • MIPS scoring and administrative rules, including the performance and data completeness thresholds holding steady at 75% through 2028, and proposed changes to how Extreme and Uncontrollable Circumstances (EUC) status is determined.
  • A new “MIPS Core Measure” designation covering 78 quality measures, replacing the old Outcome and High Priority measure concepts, plus an expanded scoring range for 17 topped out measures.
  • The sunset of traditional MIPS after the 2028 performance year, and the resulting shift to MVPs as the only reporting option for non-APM participants starting in 2029, including the 3 new MVPs proposed for 2027.
  • Updated QP thresholds under the Consolidated Appropriations Act of 2026, along with the proposed conversion factor reductions for both QPs and non-QPs.
  • Promoting Interoperability changes, including removed attestations, new Electronic Prior Authorization measures, and more.
  • MSSP and ACO updates, including where the program stands today, the Basic and Enhanced track adjustments, new Part B cost sharing flexibility, and quality reporting changes.
